New CMS Application Fee Now Affects Oklahoma Enrollment

$750 CY2026 Institutional Application Fee

CMS set a $750 application fee for institutional providers enrolling, revalidating, or adding a location in any state Medicaid program in 2026, including SoonerCare. Individual physicians and non-physician practitioners are exempt. Providers who already paid this fee for Medicare or another state's Medicaid can claim an exemption with documentation — we handle that paperwork so you don't pay twice.

Group vs. Individual Enrollment Gap
